False PHD is a passive item introduced in The Binding of Isaac: Repentance expansion that flips the game’s risk-reward pill mechanic on its head. Pill Identification: Automatically identifies the effect of all pills upon pickup.
Immediate Drops: Instantly spawns one random pill and grants one Black Heart upon picking up the item.
Negative Transformation: Converts all potentially “good” pills into their negative counterparts.
Stat-Down Damage Buff: Grants a permanent +0.6 Damage upgrade (+1.2 for Horse Pills) every time you take a stat-down pill. This effect applies retroactively to stat-down pills you consumed earlier in the run.

The item is highly valued for high-level risk management. It allows you to intentionally tank less critical stats (like Range Down or Shot Speed Down) in exchange for pure, uncapped damage.
Placebo: Combining False PHD with the Placebo activated item allows you to repeatedly trigger a specific stat-down pill to farm infinite damage, or a bad effect pill to farm infinite Black Hearts.
Harmless Effects: Pills like Addicted or Amnesia can be saved and popped right before leaving a floor, allowing you to collect the resulting Black Heart with virtually zero downside.
Regular PHD / Lucky Foot / Virgo: If you hold both PHD (or Lucky Foot / Virgo) and False PHD, pills return to spawning as both good and bad. However, taking the bad pills will still grant you the False PHD damage or Black Heart bonuses.
The Ponies: Holding A Pony or White Pony sets your base movement speed to 1.5, completely neutralizing the risk of Speed Down pills while still giving you the damage bonus.
